Lifelong Access is seeking a Job Facilitator to provide direct support to people with developmental disabilities in any environment which the individual chooses to participate, using Systematic Instruction when applicable and immersed in the Behavior Health Clinic model. The Job Facilitator will follow Individual Support Plans (ISP) for all supports provided and assist with services meant to increase independence and overall life fulfillment through the assistance of personal care, job related tasks, transportation, and skill building. Primary responsibilities include:
The working environment for this job is a changing one that adjusts to the needs of the individuals supported. The environment may at times be fast-paced with interruptions, distractions, and deadlines. Job Facilitators need to be flexible and be able to adjust to changing needs or activities. While performing the duties of this job, the employee is occasionally exposed to outside weather conditions. The noise level in the work environment is usually moderate. The employee may be required to work early mornings, evenings, and/or weekends to accommodate the needs of people being supported.

While performing the duties of this job, the employee is frequently required to stand; walk; and sit. The employee will also frequently need to converse with adults – some who might be difficult to understand. The employee is occasionally required to stoop, bend, push, pull, reach and walk for periods of time. Must occasionally be able to lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision, and distance vision.

Job Type: Full-time
Primary responsibilities include:
- Provide on-site job coaching, supporting the individual in completing duties assigned in job description, to assist individuals in maintaining community employment.
- Use Systematic Instruction to support individuals in learning and completing job tasks, completing job task analysis each time.
- Use 25% of time spent on the Behavior Health Clinic model to assist individuals.
- Develop and foster positive relationships between employer and person receiving supports that maximize the person’s independence.
- Provide job development activities, including but not limited to transportation of individuals; assist with resumes, references, cover letters, and job applications; and interview preparation.
- Foster skill-building and work towards goals laid out in the ISP.
- Maintain collaborative professional relationships with the individual and all support team members (including family/friends).
- Assist with identifying, securing, and using needed adaptive equipment and therapies (physical, occupational, speech, respiratory, psychological).
- Assist the individual with personal care, including, but not limited to, hygiene, changing, and toileting.
- Able to provide support through CPR/First Aid if necessary.
- Desire to make a difference in lives with people with developmental disabilities
- Must be at least 18 years of age
- High school diploma or GED
- CPR/First Aid certification
- Valid Illinois driver’s license with driving record in good standing
- Current proof of automobile insurance
